Lexington Two families: Complete your School Climate Surveys by April 12

Greetings Lexington Two Families,

Each year as part of the School Report Card, parents/guardians are asked to complete a School Climate Survey. This survey is open now and responses must be submitted by April 12, 2024. This is an important way for us to capture stakeholder feedback and helps us track progress toward meeting annual improvement goals on the state report card for the district and for each school. We are asking for your help as we collect this information.

To complete the survey, you will need access to your Parent Portal account. If you do not know how to access your Parent Portal account, please contact the office at your child's school.

The survey for parents and guardians will allow one submission per student. After the survey has been completed, the link will no longer work, and additional survey responses cannot be submitted.

Accessing the Survey through a Web Browser

  1. Navigate to the Parent Portal login page (https://lex2.powerschool.com/public/)
  2. Enter your username and password to log in to your Parent Portal Account.
  3. Select the Forms link from the left-hand Navigation bar. 
  4. Go to the ‘General Forms’ tab. 
  5. Click on the form titled ‘2024 PARENT AND GUARDIAN SCHOOL CLIMATE SURVEY’ to open the form.

Click on the link within the form and on the Qualtrics survey page that appears, follow the instructions to complete and submit the survey. Repeat the steps listed above if you have multiple students associated with your Parent Portal and you wish to submit a separate response for another student listed. 

Accessing the Survey through the PowerSchool Mobile App

  1. Navigate to the PowerSchool App on your mobile phone. 
  2. Enter the  ‘District Code’ (HBDX) and click Submit. 
  3. Enter your username and password to log in to the mobile app. 
  4. From the Dashboard select ‘More’ (three dots)
  5. Click on ‘Forms’ to access forms that have been shared to the student account. 
  6. Go to the ‘General Forms’ tab. 
  7. Click on the form titled ‘2024 PARENT AND GUARDIAN SCHOOL CLIMATE SURVEY’ to open the form. 
  8. Click on the link within the form to access, complete, and submit the survey.
